Additional Medicare Tax
The Additional Medicare Tax is a 0.9% surtax on earned income exceeding threshold amounts: $200,000 for single filers, $250,000 for married filing jointly, and $125,000 for married filing separately. It applies to wages, self-employment income, and railroad retirement compensation. Unlike regular Medicare tax, there is no employer match — it's paid entirely by the individual. For self-employed workers, it's calculated on net SE income above the threshold and must be factored into estimated tax payments.
Example
A single freelancer has $280,000 in net self-employment income. The Additional Medicare Tax applies to $80,000 ($280,000 − $200,000 threshold) at 0.9% = $720. This is added on top of the regular 2.9% Medicare portion of SE tax.
